2008 Camp Medical Form

The most important piece of paperwork that is required for Cub Scout Resident Camp is a correctly completed medical form.

Without a correctly completed and signed form, you can be denied access to camp.

 

If you or your scout need to schedule a physical with your health care provider...do it now...do not wait until the last minute!

Copies of completed medical forms must be turned into me by July 18th. Please be aware that I will hold completed medical forms in strict confidence until turned over to Council. 

If you print out the medical form onto separate pages and it is stapled together for submission, please put you Unit #(Pack 142), Campsite(Ledge), and name on each page.

 

So here is some additional information concerning medicals form:

MEDICAL FORMS:

 

The Camp Sequassen medical form is the preferred form you use this year. A copy is available below.

If your current Class 2 or 3 physical forms are still valid, you may use those forms until they expire. Traditional Class 2 and 3 forms will be accepted, however, Sections A,B and D, if applicable, of the Camp Sequassen form must be filled out and signed by your physician and attached to the Class 2 or 3 form. School physical forms may be accepted, but final decision lies with the Camp Health Officer and Camp Director.

 

PHYSICAL EXAMINATION:

 

  • Each Scout and parent (adult) staying in camp 24 hours or more must have a completed camp medical form on file at the Camp Health Lodge.
  • Physical examinations cannot be conducted at camp.
  • Scouts and adults under age 40 are required to have a physical examination within 3 years of the last day of camp and a camp medical form filled out and signed by their doctor.
  • Adults 40 and over: Must have a properly filled out camp medical form signed by their doctor within 12 months of the last day of camp.
  • Keep a copy of the completed health form
  • Bring an extra copy of the completed health forms to camp with you.
  • Please do not turn in the only copy of your medical form.

 

Please review the completed medical forms to ensure the following information is complete:

  • Is there a physician’s signature?
  • Is the form signed by parent or legal guardian?
  • Is the physician’s evaluation and advice section completed?
  • Was the exam conducted within the last 12 months (or within BSA date allowance)?
  • Is the applicant’s name, address and emergency contact information accurate?
  • Is an “Authorization to be Guardian,” letter attached for a Scout who is attending with an adult other than his parent?
  • Are “ALL” of the immunization dates recorded (especially tetanus and boosters)?
  • Is the history of communicable diseases complete?
  • Is the health section, with serious health conditions noted and filled in?
  • Is the applicant’s insurance information provided?
  • If taking medications, is Section D completed?
